Please fill out and submit the form below as part of the match-making process. Fields marked with * are required.
Upon submission of an approved Adoption Questionnaire, the animal’s foster parent or a staff member will contact you to answer any questions you may have, talk to you about what you are looking for in a pet, and arrange for you to meet the animal if it is determined to be a good match.
We do ask for your patience while waiting to be contacted. We have a small staff maintaining a busy organization, and fosters are volunteers with families, jobs, school schedules, etc. If you have not received a response within a week, please call our office or follow up with the relevant program coordinator at cats@leoncountyhumane.org or dogs@leoncountyhumane.org
